My experience : Painful orgasm + bleeding since August + on mini pill since May + pain in lower abdomen

trigger warning: mention of abortion

----

Hi everyone, I'm writing here to share my store and hopefully get some advice/suggestions. I would highly appreciate if any specialist could comment on my problem.

Background 1: In March this year I found out I was pregnant and decided to terminate it, I had medical abortion in May which failed and had to have a surgical one few days after. It was an aspiration procedure, 5 minutes job and it was successful. A week after I went back to ER with a lot of pain in my uterus but they didn't find anything and blamed it on post surgical thing. I started taking POP (mini pill) right after medical abortion. I had my breakthrough bleeding through June and then it was fine.

Background 2: Female, 30+ years old, overweight, no general health issues, in long term relationship with one partner.

Problem: In August I started noticing my orgasm became painful. The pain last ONLY while I'm climaxing. It feels sharp, poking, pinching, it is rather unpleasant. A week after while I was sitting on couch, I had sharp pain in my bottom right abdomen that lasted maybe a minute. I had two more pains like that one after another and then it went away. I blamed it on the POP. The next morning I woke up and I have been having a pain in my bottom right abdomen ever since. I mean all the time. The pain is located very low, in my right ovary area. Sometimes I feel it moving to inner right thigh. Sometimes I feel the pain moving left and right (still located on the right bottom side) but it always "ends up" in the same spot. It feels like something is there, like a cyst or ulcer or something blocking something. When I started having the pain, I was spotting already for 2 weeks. No pad needed and the blood was there not everyday. Only sometimes when wiping. I have been spotting all the time until today. I have been feeling very bloated and gassy. I don't have any other symptoms.

Since August I have been to 4 different doctors. Two GP and two gynecologists. First doctor thought it was appendix but tests were good and I didn't and I don't feel any pain while touching my abdomen. He said to get back to him if it persist. Two weeks later I had taken blood tests which were not so good and shown infection (22% crp and my white blood count was over 9 - beside that everything good, no uti, no sti, no bacteria) and had another appointment. The ultrasound shown my cervix was thicker than it should and I had an ulcer on the entrance of my cervix. No sign of cysts, ovaries good, uterus empty. I got prescribed two antibiotics. Two weeks later I had another batch of blood and urine tests. Blood results were great, no sign of anemia (been battling this since abortion), white blood count dropped to 7. The pain was still there so I pushed doctor for more tests. I went for another visit to GP who was stunned and didn't know what to do and what can cause it. He referred me for more tests (all clear) and visit to specialist. I have the appointment in November but I decided to visit gynecologist privately few days ago. The doctor said there is no sign of ulcer anymore and my vagina looks healthy. No weird smell nor discharge. She told me to double on mini pill for 1 month to stop spotting and possibly change the pills in the near future if nothing works. Since then (last Wednesday) I have been having some period like symptoms - some more bleeding and slight cramps in my uterus. I noticed it worsened after having a penetrative sex. I also have been very dry down there (I assume it is because of the pills + blood and had to use internal gels to ease the irritation).

I tried to see if the pain is food related but it doesn't change anything. The pain gets worse if I sit for longer time or stretch or lay on stomach. At times it feels like pulling. Sometimes it feels like ovary, sometimes not.

I'm lost. I don't know what should I do anymore since the pain is still there and doctors can't find anything. What they haven't done yet is a standard ultrasound so I will be asking for it in November. I'm also going to ask for a new pap test. The pain is my abdomen AND the pain during the orgasm is really putting a lot of strain on my mental health. I have a high libido and I love sex and this makes it very hard to enjoy anymore. I don't know if the orgasm pain is related to the abdomen pain or are those two separate issues but the timing of how they both starts make me think it is the same issue.

If anyone was ever in similar position, I would appreciate ANY suggestion.

Thank you.
