My experience : Had sex with no condom (pulled out) while she missed a pill and took 2 at once in the first week of her cycle.

My girlfriend took one pill after her placebo week (meaning the first pill in the pack). She then missed one and then the next day took 2 pills at the exact same time she normally takes a pill. We then had sex with no condom twice and I pulled out both times. Is this cause for concern? Plan b is an option but should she use it?
